**ANYONE THAT IS INTERESTED IN SETTING UP TENTS AND TABLES THE DAY OF, LET ME KNOW. **

We will have a liability waiver for a parent/guardian to sign at the party or beforehand at Denise’s house (12608 Stanwyck Circle) (if child is minor, under age 18) – – this must be signed before a child is allowed to use the slide. Link is below for advanced completion. Or text me and I can drop one off at your house. 727-644-9125. • We will provide wrist bands for kids who have had waiver signed; only those with wrist bands will be allowed to use slide (note: if you’ve already signed, we have note of that and will simply give you a wrist band day of party)

Liability Waiver Form

FOOD:

• Remember it’s HOT!! It’s recommended that you bring food that does not have to be kept cold or that can spoil – Good suggestions for the kids are pre-packaged items, like chips, cookies, gummies
• If you’re bringing anything for the kids that will be kept in your cooler, please label the cooler on the outside so they’ll know what’s in it (i.e. popsicles, watermelon)
• If you’re bringing food in your own container, please label the container and any utensil with your first/last name and street address, so we know who to return them to in event you leave behind (every year we have lots of items left behind and don’t know who to return them to!)

OTHER REMINDERS:

• Bring chairs or blankets for kids/yourself to sit, if desired (label chairs too please!)
